[patch] More cleanup of win32.h

Hi,
This patch does some more cleanup of win32.h. It defines PATH_MAX if not already and removes a duplicate definition. Also moves a struct definition out of a #ifdef _MSC_VER so that it can be used for mingw too. OK?
